The documents below are the forms for instructors to complete at various stages of the investigation and for students to complete if they wish to appeal the finding and/or sanction.
All forms are fillable Word documents and include embedded instructions to help you complete the form. When you start typing the instructions in purple font will be replaced with your responses in black font.
Forms required for all instructor-led investigations

If you conclude that they did not depart from academic integrity

If you conclude that they did depart from academic integrity, check with your Academic Integrity Administrator for any prior findings on file and then complete one of the following forms:

If you are appealing to the University Student Appeal Board, please refer to “Starting an Appeal to USAB” section on their webpage for their required form

Students are encouraged to seek advice from an informed member of the Queen's community, such as an academic advisor or program manager, or anyone whose advice they trust and value (e.g. a parent, a close relative, friend, counselor). Students are also encouraged to consult the resources available on the Office of the University Ombudsperson’s website for more information about the investigation and appeals processes.
